Ride 365 Form Instructions

About the Ride 365 Program

Ride 365 is the official mileage recognition program for current H.O.G. members. The program includes Lifetime Recognition, Annual Recognition, the Annual Chapter Challenge, and Chrome Chapter Recognition.

A key requirement of the Chrome Chapter program is that at least half of the chapter’s enrolled members must receive a minimum of two odometer readings each year.

Old Town HOG Chapter #5039 uses the Ride 365 Form to collect and record chapter member mileage readings for the OTC Ride 365 program.

Gary Stein is collecting mileage information for each chapter member. To participate, please complete and submit the Ride 365 Form.

Before You Start

If you are logged into the website, some of your information may automatically prefill on the form.

Please review any prefilled information carefully and complete any missing fields before submitting.

If you are not logged into the website, the form will not prefill. Please complete all fields manually.

Form Fields

First Name

Enter your first name.

Last Name

Enter your last name.

Member Name

Enter your full member name as you want it recorded.

Email Address

Enter the email address where you can be contacted if there are any questions about your submission.

HOG Number

Enter your current H.O.G. membership number.

HOG Mileage Program

Select or enter the applicable Ride 365 / H.O.G. mileage program information.

Year & Make of Motorcycle

Enter the year and make of your motorcycle.

Example:
2021 Harley-Davidson Street Glide

Mileage as of

Enter the date the mileage reading was taken.

Current Mileage

Enter the current odometer reading for the motorcycle.

VIN

Enter the motorcycle VIN.

Multiple Motorcycles

If you have more than one motorcycle, please submit a separate Ride 365 Form for each motorcycle.

If you purchased or sold a motorcycle during the year, please submit information for each motorcycle.

For a motorcycle that was sold, provide the ending mileage at the time it was sold.

Before Submitting

Before clicking Submit, please review the form and make sure:

• Your name and email address are correct.

• Your H.O.G. number is entered correctly.

• The motorcycle year and make are accurate.

• The mileage date is correct.

• The current mileage is entered accurately.

• The VIN is entered.

• You must submit a separate form for each motorcycle, if applicable.

The Ride 365 program does not replace the current OTC Mileage Awards.
